Close Contact at School Form
If you have received a call out from Perry High School because your child was in close contact with a student that tested positive for COVID please answer the questions below. These questions will allow us to enter the correct attendance information in Infinite Campus for your child. If you are quarantining your child please remember to have them check Google classroom for his or her work.

Positive Student Form
Please answer the following questions if your student has tested positive for COVID. Once you have completed the form please email your child's positive test results to brandie.mcneal@hcbe.net.
